The issue of discrimination on HIV sufferers

Discrimination experienced by many HIV sufferers, this is a lot due to lack of public knowledge about HIV / AIDS and ways of spreading.Discrimination occurs when negative outlooks to encourage people or institutions to treat people unfairly based on prejudice a person's with HIV status . Examples of discrimination include the hospital or prison staff who refuse to provide health services to people living with HIV and AIDS; bosses who dismiss or prejudice the status of their employees based on their HIV status, or family / society that denied them a living, or trusted living with HIV and AIDS.Such discrimination is a form of violation of human rights,discrimination can occur anywhere and anytime. Occurring in families, communities, schools, places of worship, workplaces, also place the legal and health services. People can not discriminate either in private or professional capacity, while institutions can discriminate through their policies and activities. Discrimination is actually hamper prevention efforts and treatment of HIV and AIDS, encourage marginalization of people living with HIV and those vulnerable to HIV infection.discrimination associated with the disease causing severe psychological effect on how people living with HIV and AIDS saw themselves. This could encourage, in some cases, the occurrence of depression, lack of self-esteem, and hopelessness.In many countries, laws, policies and regulations contribute to a supportive environment for HIV and AIDS prevention, support and treatment. But even if the policies and laws that support the already existing, weak law enforcement efforts lead to discrimination continues; this because often there is little accountability for the actions of discrimination or indemnity for those who have experienced stigma and discrimination. Various countries and institutions can also create and enrich the stigma and discrimination through laws, regulations, and policies that impressed mendiskrimasi People living with HIV and AIDS or the people around him.

UN General Assembly in special session on HIV and AIDS called for to combat stigma and discrimination. TB patients in Indonesia will continue to rise

World Health Organization (WHO) estimates that in Indonesia each year will be 583 000 new TB cases. Half or 50 percent of that amount will be transmitted as it is known as Basil the case of Acid Resistance (AFB) positive. Transmission of TB disease is very fast and every one smear positive TB patients could transmit the disease to 10 to 15 people per year. Thus you can imagine how many pesakit TB in the next few years, saw the number of Indonesian population which has a population of 210 million inhabitants. Today Indonesia is the diseased state (TB), the largest number three in the world after India and China. TB disease is not a new disease in Indonesia, the TB problem, nor merely a health problem. Tuberculosis is also very closely related to economic and social problems in the country. TB patients are mostly derived from the poor and the productive age group.

TB disease is a disease that is categorized in infections caused by bacteria Mikobakterium tuberculosis. TB disease can strike at anyone , men, women, old, young, rich and poor as well as anywhere. In Indonesia in particular, this disease continues to grow each year and currently reaches 250 million new cases of which 140 000 resulted in death. Even Indonesia, the world's third largest country in the world in this TB problem.

Population growth in Indonesia TB disease is caused by the worsening socio-economic conditions in Indonesia, ineffective public health service facilities, the increasing number of people who do not have a place to stay and the epidemic of HIV infection. It also must have a major impact from a weak body resistance / decreased, the virulence and the number of germs that plays an important role in the occurrence of TB infection.

Causes of TB Disease

TB disease is caused by bacteria Mikobakterium tuberculosis, rod-shaped bacteria and is resistant to acids also known as Acid Resistant Trunk (BTA). Type of bacteria was first discovered by a man named Robert Koch on March 24, 1882, To the memory of his services was given the name of the bacterium bacillus Koch. Even TBCpada lung disease was also known as Koch Pulmonum (KP).

Symptoms of TB Disease

Symptoms of TB disease were classified into two parts, the general symptoms and specific symptoms. Difficult to detect and diagnose TB is caused by a clinical picture of the patient are not typical, especially on new cases.

1. Common symptoms (Systemic)

The spread of HIV in the world increases, the number of AIDS patients is increasing every year. even many babies are born already carry the HIV virus in their blood, increasing the spread of HIV is caused by several factors as follows:

   1. Having Sex with HIV patients or people who having HIV.
   2. Have multiple sexual partners.
   3. Having Sex with sex workers.
   4. Sharing needles, a few people use the same needle for piercing, drug use, or create a tattoo.
   5. Victims of sexual violence, for example due to being raped by HIV patients.
   6. Having other sexually transmitted diseases such as herpes, chlamydia, gonorrhea, Trichomoniasis, or

hepatitis.

7. Vulnerable women who suffer with HIV who transmit HIV to the child conceived.

HIV is slowly lowered in the cells of the immune system (CD4) of normal CD4 levels for 1000. During 5-7 years CD4 count will continue to decline until it reaches below 200 and cause symptoms.

if someone has the HIV virus, will get the following signs:

Phase 1.
In the early stages of HIV infection is usually not visible symptoms. A person can have HIV for years without knowing it. Blood tests will show antibodies after the virus is formed in the fight against the AIDS virus. However, it also takes up to three months before antibodies are formed. Phase 2.

Phase 3.

This is the phase of a person suffering from AIDS. In addition, patients can also be exposed to a type of cancer called Kaposi's sarcoma (cancer of blood vessels). Not all infants born to HIV positive mothers will be infected with HIV as well. If 100 HIV-infected mothers each gave birth to one baby, on average, 30 babies will be infected with HIV. Average virus will be transmitted in five

babies during pregnancy, another 15 at the time of delivery, and 10 babies through ***

milk. Therefore, it is very important for every pregnant woman to determine whether she is

HIV positive or not (especially for those whose lives are at high risk for contracting HIV / AIDS).

3. Counselling is an important component of the response to the epidemic AIDS. People who are infected or affected by HIV, need information, advice and support to overcome the situation.

Does anyone have a good tool, program, or software they have already developed to keep track of when all the various reports are due? I am certain that we do not have to reinvent the wheel on this one.

The International HIV/AIDS Alliance (Alliance) and the International Council of AIDS Services Organizations (ICASO) are conducting an online consultation for civil society to input into the draft framework that has been developed to outline community systems strengthening (CSS for short). The Global Fund to fight AIDS, Tuberculosis and Malaria has developed the CSS Framework in collaboration with other stakeholders to make it clearer the range of community strengthening activities the Global Fund is mandated to support.

We want your views on this framework, which will play an important role for anyone working at a community level.

Filling the gaps

The CSS framework has been developed in recognition that there are gaps in funding for many aspects of community action on HIV, TB, malaria, reproductive health and other health-related issues. It has been developed specifically to support the CSS component of Global Fund grants, but is applicable to all community based activities aimed at improving health though community based action.

By feeding back on the draft framework, you will help further develop what will become a key document which can:

Help civil society actors articulate and scale up their activities, and access Global Fund funding
Equip governments to better understand the vital roles of community actors particularly when building stronger health systems
Enable the Global Fund to make informed decisions about awarding grants to proposals strong in CSS.

Background information

The work to develop the CSS framework builds on two years of consultations and input gathered at regional and national meetings. These consultations included a global meeting on TA for CSS held in Bangkok in June 2009. This background work lead to the articulation of CSS in a Global Fund fact sheet and a UNAIDS guide on CSS which was used by many groups in developing Round 8 and 9 proposals.

The Technical Working Group (TWG) has been charged with developing a framework which among other things distinguishes CSS from HSS and clarifies how CSS fits into the performance measurement system of the Fund. In addition to reviewing several drafts of the CSS framework document the TWG oversaw nine field visits which were designed to help develop a Monitoring and Evaluation framework to be used in measuring CSS work.

The TWG will take into account feedback from the online forum and the Brighton meeting (March 18-19) to finalize the framework.

Following is translation of an e-mail generated by an employee of International HIV/AIDS Alliance – Global Fund’s Principal Recipient in Ukraine - to its sub-grantees in Dniepropetrovsk region of Ukraine. Original version is following the translation.

Hello colleagues!

I am sending to you the scanned documents (attached at the bottom of this document) that were discussed in a meeting of Paola Pavlenko (Risk and Grant Compliance Manager of International HIV/AIDS Alliance) on December 8, 2009. Your attitude to the content of these letters and to position of Board of Coalition of HIV/-service organizations should be preferably expressed in writing, and no later than December 11, 2009. It should be addressed to the author – Elena Davis (davis@hiv.org.ua), with a copy to harm reduction association (uhra@ukrpost.ua, botvin@uhra.org.ua) and to me. I think this should contain opinion about what the author accuses us of – presumed data falsifications in reports, which NGOs are presumably forced to make, illegal schemes of financing projects that are presumably contained in sib-grantee agreements with Principal Recipient (by which is meant International HIV/AIDS Alliance).

It would be interesting if you expressed your opinion whether you need a unified all-Ukrainian electronic database of our clients, as well as on the ability of your NGO to deliver services to other vulnerable groups, besides those contained in Global Fund indicators, with which you are working now.

Is everybody in agreement with the statement that NGOs are doing imitation and formal organization of project process?

Does lack of social protection affect rotation of personnel in your projects?

Do you need any other body (government, non-government) that would monitor your work?

Do you think HIV-service Coalition can be trusted with a watch-dog role towards the project of Global Fund?

On December 14, 2009, there will be a CCM meeting where these documents will be discussed and therefore your voice and your opinion can play an important role.

Ваше отношение к содержанию документов и вообще к позиции Правления «Коалиции ВИЧ-сервисных организаций» желательно высказать в письменном виде, не позднее 11 декабря и направить автору - Елене Дэвис (davis@hiv.org.ua) с копией на ассоциацию снижения вреда (uhra@ukrpost.ua, botvin@uhra.org.ua) и мне. Думаю, стоит написать автору об обвинениях в ваш адрес о приписках в отчетах, которые якобы вынуждены делать НПО, мошеннических схемах финансирования, прописанных в типовых договорах с Основным реципиентом (имеется ввиду Альянс).

Исходя, прежде всего, из интересов населения Украины, а особенно наиболее уязвимых к эпидемии ВИЧ/ СПИДа сообществ: потребителей инъекционных наркотиков; работников коммерческого секса; ВИЧ-инфицированных; ЛГБТ; осужденных; лиц, освобожденных из мест лишения свободы; мигрантов и других слоев населения, а также с целью обеспечения эффективности программ профилактики ВИЧ/СПИДа,

ВБФ «Коалиция ВИЧ-сервисных организаций» (далее Коалиция) выражает обеспокоенность относительно ситуации, сложившейся в сфере ВИЧ-сервиса.

Существующая практика работы донорских международных и национальных организаций в Украине не дает возможности оценить реальный охват услугами представителей наиболее уязвимых к эпидемии ВИЧ/СПИДа групп населения, и, поэтому, многие из них остаются за пределами сервиса, который им крайне необходим.

В последнее время, в ВИЧ-секторе Украины ведется острая дискуссия. Основной вопрос, нужен ли независимый мониторинг и контроль за деятельностью проекта Глобального Фонда «Поддержка профилактики ВИЧ/СПИДа, лечения и ухода для наиболее уязвимых слоев населения в Украине» (далее - Проект ГФ) Должно ли общество контролировать расходование денег по Проекту ГФ для достижения честных и реалистичных целей в борьбе с эпидемией ВИЧ/СПИДа?

В этой связи Правление Коалиции считает необходимым ВЫСКАЗАТЬ СВОЮ ПОЗИЦИЮ по данному вопросу.

В сфере ВИЧ-сервиса в Украине возникли системные проблемы, на которые нельзя закрывать глаза гражданскому обществу.

Основные проблемы:

1. Невозможность определить реальное влияние на эпидемию мероприятий, осуществляемых в рамках проекта ГФ:

- Главным критерием оценок результативности и эффективности проектов признается большой объем спорных количественных данных, за которыми теряется конечный качественный результат;

- Отчетные данные по охвату целевых групп проектными услугами, предоставляемыми неправительственными ВИЧ - сервисными организациями (далее - НПО), не всегда совпадают с альтернативными данными;

- Региональные НПО вынуждены осуществлять приписки или недостоверно фиксировать степень охвата клиентов во имя обеспечения требуемых основным реципиентом (далее - ОР) индикаторов, становясь, таким образом, де-факто и де-юре участниками мошеннических схем, закрепленных в типовых договорах с основным реципиентом;

- Отсутствие экспертов, независимых от финансирования ОР, способных провести анализ эффективности проектов, как на региональном, так и на национальном уровне.

2. Незаинтересованность ОР в эффективной работе Национального Совета по вопросам противодействия туберкулезу и ВИЧ/СПИДу (далее НС) в связи с нежеланием реального контроля над своей деятельностью:

- Сосредоточение стратегического и технического руководства Проектом ГФ в руках ОР при отсутствии надлежащего контроля и работы со стороны НС;

- Превращение региональных НПО из организаций гражданского общества, представляющие интересы своих целевых групп, в подрядчиков, обслуживающих корпоративные интересы ОР;

- Отсутствие единой национальной электронной базы данных людей, живущих с ВИЧ;

- Игнорирование интересов тех уязвимых групп, которые не оговорены в индикаторах Проекта ГФ.

3. Невозможность оперативного пересмотра индикаторов Проекта ГФ в соответствии с изменениями эпидемиологической ситуации в сфере ВИЧ/СПИДа в Украине:

- Формирование стратегии программ снижения вреда и оценка их эффективности, исходящих из объемов закупок шприцев и презервативов, а не из оценки ситуации на постоянно меняющейся наркосцене того или иного региона;

- Направленность проектной деятельности не на решение проблем по существу, а на имитацию их решения и формальную организацию проектного процесса, под который выделяются средства;

- Нежелание ОР прислушиваться к информации, поступающей от организаций- субреципиентов (далее - СР) и исполнителей проектов;

- Отсутствие площадки для открытой дискуссии между СР и ОР.

4. Неустойчивость и неудовлетворительное качество ВИЧ-сервисов на местном уровне:

- Текучесть кадров среди социальных и аутрич работников, работающих на местном уровне, вследствие их социальной незащищенности и отсутствия социальных пакетов;

- Непрозрачность и закрытость мониторингового процесса со стороны ОР и как результат, беззащитность региональных НПО - СР проекта ГФ - перед псевдообъективными решениями уполномоченных сотрудников ОР.

Исходя из вышеизложенного, Правление Коалиции ПРЕДЛАГАЕТ следующие изменения, как на государственном уровне, так и в рамках деятельности Коалиции:

На государственном уровне:

1. Создать единую всеукраинскую электронную базу данных людей живущих с ВИЧ и получателей услуг из числа представителей уязвимых групп;

2. Разработать и внедрить механизм национального контроля, который предусматривает государственный контроль и общественный мониторинг реализации Проекта ГФ с привлечением независимых экспертов для объективной оценки противодействия эпидемии ВИЧ/СПИД;

3. Пересмотреть индикаторы Проекта ГФ с учетом изменения ситуации на наркосцене и ограниченности ресурсов в стране;

4. Инициировать пересмотр типовых договоров, заключенных между ОР и СР, и привести их в соответствие с украинским и международным законодательством;

5. Разработать и внедрить апелляционный механизм как часть процесса внедрения Проекта ГФ в Украине; создать этический комитет при НС во избежание в дальнейшем конфликтных ситуаций между ОР и СР;

6. Создать профсоюз ВИЧ-сервисных НПО в целях защиты интересов их работников перед работодателями.

На уровне Коалиции ВИЧ-сервисных организаций:

1. Предложить ВИЧ-сервисным НПО определиться со своей позицией по отношению к Политической позиции Правления Коалиции ВИЧ-сервисных организаций;

2. Провести ротацию представителя НПО в Национальном Совете по вопросам противодействия туберкулезу и ВИЧ-инфекции/СПИДу с последующим выходом из состава членов НС исполнительного директора Коалиции ВИЧ-сервисных организаций Натальи Подлесной, ввиду невозможности представления интересов политически и принципиально разновекторных НПО;

3. Коалиция ВИЧ-сервисных организаций объявляет о завершении с июля 2010 года ее деятельности как СР по Проекту ГФ и о решении действовать как независимая организация по принципу «watch-dog», работая с другими проектами и программами на политическом, адвокационном и мониторинговом уровнях.

Corruption/Theft Fueling HIV and AIDS in Uganda

Widespread HIV and AIDS in fishing communities is threatening the fight against the epidemic and making the fisheries industry at crossroads in Uganda. Please, help out!!!!

It is an open practice in fishing communities, “Fish for sex” relationship and alcoholism is fueling HIV infection and making the management of AIDS in hospitals around lake shore areas in Uganda difficult and beyond the available capacity.

This is also increasingly exacerbated by complacency and widespread corruption in government institutions responsible for HIV/AIDS programs. The case in point for Uganda, is, since the Global Fund list for grantees were posted by Uganda AIDS commission (UAC) in newspapers some twelve months ago, the funds have never been released to respective NGOs/CSOs -what do you call this? “Corruption or Theft?”

Could responsible players provide any guidance and /or some light-information as to what actually happened regards the Grants!!. This state of affairs is further exacerbated by lack of transparency, indifference and negative attitude of those engaged in HIV/AIDS programming in government departments and/or ministries who are actually responsible for escalating trends of HIV/AIDS epidemic in Uganda and the fisheries subsector in particular!!!.

This has left the poor, marginalized and underserved fisherfolk unable to access HIV/AIDS related services in a situation where HIV/AIDS prevalence rates are three to five times the national average of 6.7%. It should be noted that HIV/AIDS in fishing communities has reached unmanageable levels-this is a time bomb yet to explode, which can be catastrophic indeed!!. It thus calls for urgent and discrete intervention by all players at all levels.

Through the fish for sex procurement process, HIV/AIDS is wiping out families and/or generations and fueling numbers of orphans, widows and households headed by grandmothers, grandfathers and children without a future to talk of!!!

I write to seek for any formidable redress of this situation before it completely goes out of hand !!!

Diagnosing tuberculosis in Nigeria

Tuberculosis is an extremely important zoonotic disease in Nigeria but yet the only method of diagnosis is acid-fast test which does not differentiate pathogenic mycobacteria from other mycobacterial species and even other acid-fast organisms. I am currently working on tuberculosis in cattle and cattle owners with emphasis on the disease in nomadic pastoralist but my greatest challenge is where to culture my samples because I am interested in species identification. I contacted a private lab but I was not successful, please I need help in this aspect because my work is hanging.

Kick Malaria Out (KMO) 2009 West Africa

Campaign KMO outreach will run for two weeks by the combined volunteers as part of a month long KMO campaign by VPWA and it grass root partners through out the following countries: Togo, Benin, Nigeria, Ghana, Ivory Coast, & Liberia.

Campaign KMO will start in Ghana with Volunteers from all over the world, including professionals from all fields, students etc. Volunteers will receive a training, conducted by VPWA’s Executive Director, to facilitate cultural understanding and to teach Volunteers how to address each community i.e. community customs, habits, etc. Volunteers will then be divided into two groups (Volunteer Group A & Group B). A Volunteer Team Leader will be assigned for each group of volunteers.

Between 20th August 2009 and September 3rd 2009, Volunteer Group A will travel to the countries east of Ghana i.e. Togo, Benin and Nigeria. Volunteer Group B will travel to the countries West of Ghana i.e. Ivory Coast and Liberia. VPWA partner organizations in these countries, will be awaiting the arrival of VPWA volunteer groups to facilitate any community needs. Each group will:

Conduct community workshops on Malaria by disseminating people-friendly statistical information on the spread of Malaria and providing preventative measures to prevent being infected by the disease

Conduct clean-up training exercises designed to show communities how to prevent and clear water stagnation which is the breeding source of mosquitoes and caused by a) uneven floor b) clogged drains c) poor gradient of gutters and drains. e) rain accumulation f)overflows g)leakages from pipes h)roof gutters chocked with leaves or silt i)misalignment of rainwater downpipes with elbow joint, resulting in blockages. j) Collected water in discarded receptacles.

ADDITION OF VIRAL HEPATITIS TO DISEASES FUNDED BY GFATM

We, African medical experts in liver diseases, involved in the diagnosis, prevention, treatment and care of Hepatitis B & C in Africa, meeting on the occasion of the Second Hepatitis B and C - The African Experience Exchange Conference, in Mauritius from 2^nd - 4^th April 2009;

CONCERNED about the increasing burden of hepatitis B and C virus infections, cirrhosis and liver cancer in Africa;

AWARE that some African countries are making various efforts to improve access to diagnosis, prevention, treatment and care of hepatitis B and C virus infections and their sequelae but that these efforts still remain limited and costly;

RECALLING that African leaders have made commitments as contained in the Abuja Declaration and Plan of Action on Roll-Back-Malaria of April 2000, the Abuja Declaration and Plan of Action on HIV/AIDS, Tuberculosis and Other Related Infectious Diseases of April 2001 both of which were reaffirmed in the Maputo Declaration on Malaria, HIV/AIDS, Tuberculosis and Other Related Infectious Diseases in July 2003, the AU/NEPAD Health Strategy of 2003, and AU Assembly Decision 55(IV) on the Interim Report on HIV/AIDS, TB, Malaria and Polio of Abuja, January 2005, to increase access to care and treatment of preventable diseases;

FURTHER RECALLING the recommendations of the International Conference on Population and Development (ICPD), United Nations General Assembly Special Session on AIDS (2001) and on Children (2002) and the commitment to the attainment of the Millennium Development Goals (MDGs) adopted in the Millennium Declaration 2000;

RECOGNISING the leadership provided and efforts made by the World Health Assembly through the WHO and the Global Programme for Vaccines and Immunization as shown in documents such as WHA45.17, the WHO position paper on Hepatitis B Vaccines (Weekly Epidemiological Record No. 28, 2004, 79, 255–263), Expanded Programme on Immunization. Global Advisory Group – Part I.

Weekly Epidemiological Record, 1992, 67: 11-15 and Introduction of hepatitis B vaccine into childhood immunization services (WHO/V&B/01.31).

ACKNOWLEDGING the commitments made by the G8 and other development partners;

NOTE that most hepatitis B virus (HBV) infection in Africa occurs in the age group 0-5 years, from mother to child transmission (risk of transmission ranges from 30 to 90%) and transmission during early childhood, and this results in increased chance of chronicity of up to 90% of such cases, thus perpetuating the high prevalence on the continent. HBV is 100 times and hepatitis C virus (HCV) 10 times more infectious than HIV and both lead to liver cirrhosis and liver cancer, the prognosis of which is very poor, with people dying in their prime of life resulting in loss of valuable human resource and labour force. Liver cancer is the commonest cause of cancer in males and the third cause of cancer in females in many African countries and HBV is responsible for at least 75% of this cancer.

FURTHER NOTE that the best intervention for the control of HBV infection is universal HBV immunisation and that some African countries have incorporated this in their National Programme of Immunisation. However, there is need for an aggressive implementation strategy to increase coverage and to prevent future transmission of infection. However, the strategy of immunisation alone will take decades to achieve the control and ultimately the eradication of hepatitis B and its sequelae unless it is combined with treatment and care of those presently infected. At the moment Africa has a very large reservoir of HBV infected people who are at risk of dying prematurely from the complications of HBV and HCV infections and, for women of reproductive age, in transmitting the infection to their children. Treatment of these infections are very expensive, not affordable and not readily available in most African countries;

FURTHER NOTE the high prevalence of hepatitis B and C in areas of Africa with violence and displacement of people;

RECOGNISE the leadership of the WHO Regional for Africa in providing direction for cancer prevention and control in Africa (Document AFR/RC58/4 24 June 2008) and that hepatitis B diagnosis, prevention, treatment and care provides a great opportunity to achieve the aims of the AFRO cancer prevention and control strategy;

CONCERNED that whereas HBV and HCV are more prevalent and more infectious than HIV, we observe that African governments and the world, in general, have not paid the required attention to the ravages of HBV and HCV infection on the continent. Ministries of health do not have programmes for the diagnosis, prevention, treatment and care of hepatitis B and C in most African countries;

FURTHER CONCERNED that the drugs for the treatment of chronic hepatitis B and C, even in cases where the same drugs are used to treat HIV/AIDS, are far more costly than antiretroviral drugs and far more difficult to obtain their generic formulations;

ALARMED by the persistently high levels of infection of hepatitis B and C on the African continent (life time risk of infection >60%), high levels of childhood infection, maternal to child transmission of infection, high prevalence of chronic hepatitis B and C infections (>8%), high morbidity, mortality and disability due to these preventable and curable conditions;

FURTHER ALARMED by the high prevalence of cirrhosis, liver failure and liver cancer resulting from hepatitis B and C infections in young adults in their productive age on the African continent;

CONVINCED that the MDGs and the Vision and Mission of the African Union and the objectives of the New Partnership for Africa’s Development (NEPAD) on our continent cannot be achieved without urgent and concerted effort at controlling hepatitis B & C infections in the African continent;

WE HEREBY:

Support the World Hepatitis Day celebrations;

FORM an African Expert Committee in conjunction with the African Association for the Study of Liver Diseases (AASLD) and the Association Africaine Francophone de Formation Continue en Hepato Gastro Enterologie (AAFFCHGE) to:

Continue to monitor the magnitude of the problem of hepatitis B and C on the African continent;

Continue to monitor the World’s and African response to this ongoing catastrophe;

Review the current international guidelines for the management of HBV and HCV and modify them to suit the local African situation;

Provide a frame work for action and

Continue to commit ourselves to the promotion of best practices in the management and prevention of Hepatitis B & C in Africa

REQUEST the World Health Assembly to include the diagnosis, prevention, treatment and care of hepatitis B and C on the world health assembly agenda;

REQUEST the World Health Organisation to:

Develop a strategy for accelerated hepatitis B and C diagnosis, prevention, treatment and care in the African Region;

Provide technical support to Member States in the development and implementation of action plans for acceleration of hepatitis B and C diagnosis, prevention, treatment and care interventions;

Mobilise more resources and long-term international support for scaling up effective hepatitis B and C diagnosis, prevention, treatment and care;

REQUEST the Board of the Global Fund for the Fight against AIDS, Tuberculosis and Malaria to include the diagnosis, prevention, treatment and care of hepatitis B and C in the diseases to be funded by the Fund;

URGE African states to

Scale up the diagnosis, prevention, treatment and care of hepatitis B and C using proven effective medicines and vaccine; in this wise we urge the implementation of universal immunisation with Hepatitis B vaccine and ensuring the widest possible coverage including catch-up immunisation of children of school-going age, health workers and family members of hepatitis positive people and all those are high risk of infection

Make efforts to reduce the costs of medicines for the diagnosis, prevention and treatment of hepatitis B and C by waiving taxes and other charges on them and ensuring their populations have access to low cost but effective medicines;

Strengthen Health Systems to promote universal access to hepatitis B and C treatment and care by implementing the Abuja Recommendation of allocating at least 15% of the national budget to health;

Create national hepatitis B and C control programmes to co-ordinate and implement evidence-based best practices to control and eventually eradicate hepatitis B and C infections;

URGE pharmaceutical companies manufacturing medicines for the diagnosis, prevention, treatment and care of hepatitis B and C to:

Reduce the prices of these medicines to the same level as those for HIV/AIDS and

Develop country programmes to support patients on treatment

URGE the International Community, the G8, G20 and other donor countries to

Fulfil their commitment to honour their pledges to ensure availability of resources for sustainable access to treatment and care of hepatitis B and C for the achievement of the millennium development goals in Africa.

Increase funding to African countries to cover this new initiative of improving the diagnosis, prevention, treatment and care of hepatitis B and C.

So politicians are the problem! Our cities and local governments had a programme on mosquito control. All this has died away, at least I don't see any such action in Uganda. May be the Global Fund should insist on this cheap and cost effective method. It works. communities can be mobilised to do the job. Problem is money has spoilt every one. No more self-help drives. It may also not be fair to expect governments to do everything. The people must rise up and support.

Why don't we start fighting the mosquitoes by destroying stagnant waters? I remember when most African countries got their independence in the early 60s the basic public health measure were applied which was to fight the mosquitoes anywhere they can be found insecticides were used either by plane, my vehicle, foot or motorbike even in the remote areas. But few years down the line, because of all the problems we all know created mainly by the politicians, we are all waiting for the Western to fight Malaria for us, all the strains are now resistant to most of the molecules and as you mentioned the net is not the solution because Mosquitoes can be found everywhere. Sometimes the simplest method can be more efficient.

Education of the population on how to maintain their environment clean, by destroying and avoiding putting used water everywhere, clean the herbs around the houses and old tires etc can be a very good start.

Dear colleagues,

The rate of infection of malaria in Uganda does not appear to be reducing in practice. Peasants do not appear to have the money to afford mosquito nets. and even then who said mosquitoes only attack sleepers. peasants are exposed to malaria through various means.

from your perspectives and experiences what could be the chances of eradication through biological means. too much bush, stagnant waters. Are there mosquito eating insects. As a financial expert am interested in evaluation of costs for this alternative.

Dear Friends and Colleagues,

I am an independent consultant currently working on a strategic paper on the India Country Coordination Mechanism (I-CCM) reconstitution process. The I-CCM is a "national consensus group" which has been operational in response to the requirements and recommendations of the Global Fund to Fight AIDS, Tuberculosis and Malaria (GFATM) as an overall guiding body for the use of Global Fund grants.

The Global Fund recognizes that through a country driven, coordinated and multi-sectoral approach involving all relevant partners, additional resources can have a significant impact on the reduction of infections, illness and death from the three diseases. The mandate of the I-CCM is to:

1. Solicit, evaluate and approve proposals to be included in the Comprehensive Country Proposal of the Global Fund;

2. Provide planning and other support for implementation of the programmes funded by the Global Fund; and

3. Monitor all the programmes funded by the Global Fund.

Central to the Global Fund's commitment to local ownership, multi-stakeholder representation, and participatory decision-making, is the I-CCM membership pattern. The Civil Society Organisation (CSO) Constituency in the I-CCM is a representation of the organisations working for people living with or affected by the three diseases, namely, HIV, Tuberculosis and Malaria. The two-year term of the CSO members is ending early 2009 and the election of new Constituency Representatives to the I-CCM is to take place during March-April 2009.

Specifically, I request suggestions and recommendations from the members of Solution Exchange, on:

What should be the selection process followed for the upcoming re-constitution of the I-CCM?
What methods would you suggest for increasing the participation levels of all relevant partners?
What are the ways by which we could increase the transparency and credibility of the processes followed by the I-CCM?

Your inputs will go into the strategic paper being prepared, which would be shared with the authorities and the community once it is finalised. More importantly, the deliberation will help to bring about a broader and inclusive participation in the CCM especially in the CSO constituency members’ selection process.

Re-Identifying problems in grant implementation

Iwould say I just joined myGlobalFund e-forum just recently and I haven't contributed much, but I like reading your contributions. I Fenna's contributions give me the courage to write and say what is happening exactly under my nose, but failing to disscuss or share with anyone.

In Zimbabwe we receive GF funds for round 5 it has never been implemented. I work in a National TB Reference Laboratory where almost most al the laboratory activities should be. But since 2005 there was no personel recruited for the GF post but after 3 years since we were awarded the funds they have recruited personel who can not perform any TB smear microscopy . TB culture, drug susceptibility test, including quality assurance but are to be sent now outside the country to do the training. Is it not that you recruit those who will implement the program and have reports done for what you have implemmented in between intervals.

These are some of the abnomalities that are required for the ccm to monitor and see whether in recruiting to implement the program the correct staff has been in place. If they know the have applied for funding as they is a lot of nepotism and corruption they should send these have people in place before they start implementation. In my country the TB program is at standstill as there is a lot of nepotism and political inteference including nepotism, corruption and no fairness and no transparency in conducting the implementation of the program. SP also do not they roll and they have not implemented anything. We are in a crisis. I am seriously outraged in stopping TB but I am now a spectator failing to do anything as the political interference is in its highest.

Negotiating marginalizing: Gender beyond dichotomy

While I was filling up my profile, I found Gender as usual has two options: Male and Female. I have not filled up my gender as a token of my "protest" against such mistake done by TGF. We are aware that the Global Fund (TGF) has shown its respect for sexual minorities. I think to further strengthen this commitment and to acknowledge the sexual and gender rights of all human being as their basic human rights, TGF can take up the initiative to have a third option under gender category beyond Male and Female in order to allow people to choose and write their gender in their preferred way.
